  Keep in mind that all 7th grade students will now be required to get the Tdap vaccination prior to the beginning of their 7th grade school year.  Tdap stands for tetanus, diptheria, and pertussis which helps cut down on the whooping cough cases in schools.

3- 5 At least one dose must have been administered on or after child's 4th birthday.  3 doses required for grades 1-12; 4 doses required for Kindergarten students; an additional dose is now required to enter 7th grade.
Measles
(M, M/R, MMR)
2 1st dose must have been administered on or after child's 1st birthday; 2nd dose has to be given at least 28 days after the 1st dose. 2 doses required for all Kindergarten-12th grade students.
Hepatitis B 3 3 doses required for all Kindergarten, 7th grade, and transfer students.
Varicella
(chickenpox)
1 or 2 Two doses required for all Kindergarten students; 1st dose must have been administered on or after child's 1st birthday and then the 2nd dose must have been administered at least 28 days after the 1st dose.  Students under 13 yrs. must have 1 dose.  Students over 13 yrs. must have 2 doses seperated by 28 days.  A history of the disease from parent/guardian or physician may be accepted in lieu of the vaccine.
